Please use this form to request Epic access for a new non-provider end user, request an update to current non-provider end user access, or terminate any end user access. This applies to both inpatient and ambulatory environments.
Examples:
- A nurse working at an inpatient unit.
- A front desk person who works at a doctors practice.
- Terminate ANY users access.
*For New Multiple Non-Provider Users, please attach a copy of the Multiple User Spreadsheet w template, on the right side of this service, to the ticket. Spreadsheet MUST be filled out completely and attached to a ticket to complete the request.
